About I-PATH Community Health Needs Assessment Project

VIDEO: Loyola I-Path CHNA Overview  The Loyola I-Path is a graduate program that promotes inter-disciplinary collaboration pertinent to public health. The Community Health Needs Assessment portion of the I-PATH provides guidance to non-for -profit hospitals in fulfilling their CHNA requirements pre new guidelines instituted by the Affordable Care Act.

A Community Health Needs Assessment (CHNA) is the process for a hospital to evaluate the health needs of the community it serves. The CHNA process supports prioritizing community health needs and devising strategies to address them. (See Background on Community Health Needs Assessment.)

The Loyola I-PATH Community Health Needs Assessment (CHNA) Project is aimed at providing students and practitioners in the health care professions a greater understanding of the CHNA purpose, process and outcome. Our CHNA portal is a gateway to valuable information such as links to federal guidelines, Q and A on the CHNA process, tips on methodology and model CHNA reports.

In particular, information provided by the I-PATH project can be helpful to hospitals with limited resources to devote to the CHNA process, including those associated with Medically Underserved Areas/Populations (MAU/Ps). (See Medically Underserved Areas/Populations.)

Valuable information presented by the I-PATH project includes a set of eight learning modules. The modules are based on materials delivered in the Loyola University Chicago graduate course on Health Care Program Planning & Evaluation. These materials focus on the methods of CHNA as well as its use as a health care program planning tool.
