As part of this grant and educational journey, we will learn about the patient centered medical home and interprofessional collaborative practice. This educational experience will consist of several topics and methods of learning. 

WHAT IS I-CARE-PATH? 

This educational program is supported by a grant from the Health Resources and Service Administration (HRSA). The grant is called the Interprofessional-Collaborative Redesign and Evaluation for Population Access to Health (I-CARE-PATH), which is described under the tab “About Us.”

WHAT IS A PATIENT CENTERED MEDICAL HOME? 

As patients transition from one setting and one provider to another, patients are becoming confused about what to do about their health and health care. Patients are lost between acute, ambulatory, and community care settings. A new delivery model, called the Patient Centered Medical Home, is a strategy to partner with patients to navigate the health care system based on the patient's health care goals.

WHAT IS INTERPROFESSIONAL COLLABORATIVE PRACTICE? 

The Majority of these learning modules will relate to Interprofessional Collaborative Practice, or IPCP. There are three modules, addressing the following four competencies under the tab "Interprofessional Education Learning Modules":
